Just like in any other business online casinos are run with a focus on keeping certain groups of players more happy then others. There's no doubt that a player like in your example Angie who spends 100K a month is going to get more from an online casino then one who spends a significant amount less...
I've heard of situations where players from the US were flown down by various Microgaming casinos in the past (of course when Microgaming was still allowing their operators to accept wagers from US players) to places like Monocco, Dubai and other destinations around the world where the owner or those with interest in the casino operation would meet with them and basically pamper them for a week or two... I think that casinos need to do whatever they possibly can to retain these monster players and there's no question that in certain cases they'd be willing to spend a whole lot more in keeping the player around.
I think this is also further by the reputation a player holds at an online casino. A player who doesn't complain and enjoys gaming, doesn't charge back any transactions or make for any problems at the casino is more likely to do better in terms of the rewards and incentives they're offered.
